How do I obtain a JTS disabled persons' ID card?
First, obtain a typewritten letter from your physician, Social Service agency, or health care professional on their office letterhead stating your disability. Bring that letter with proof of identification to the Clerk/Treasurer’s office on the first floor of the Municipal Building at 18 N. Jackson Street. A JTS Disabled Person's ID card will be prepared for you, at no cost, while you wait.
